IMMUNIZATION/VACCINE OVERVIEW

The table below provides an overview of the vaccines required by the state of Illinois for school attendance in grades K-8. Parents may also submit a Religious Objection form for any or all immunizations. Students who do not provide proof of the required immunizations or a Religious Exemption form by the deadline will be excluded from attending school on October 16th or the first school day after the deadline until the required information is received.  

Please contact your school nurse if you have any concerns or extenuating circumstances regarding immunizations.
