Health Examinations and Immunizations
The state of Illinois mandates the following health examinations for students:
- Kindergarten
- 2nd Grade
- 6th Grade
- 7th Grade - if participating in after school sports
- 8th Grade - if participating in after school sports
- Transfer students from another state or country

Certificate of Child Health Examination Form and immunization records:
- Physical examinations must be conducted within twelve months prior to the first day of school.
- Parents must complete and sign the health history portion of the health form (see example below).
- For students transferring from another state or country, a health form that is comparable to the Illinois requirement is acceptable.
- Completed forms are due no later than October 15th of the school year. Students who do not provide the required health forms and proof of the required immunizations by the deadline, will be excluded from attending school on October 16th or the first school day after the deadline until the required information is received.
- In lieu of presenting a health form or updated immunization records, parents may submit a Certificate of Religious Exemption form. This form must be completed and signed by a parent/guardian and the child’s health care provider. Updated exemption forms are also due by October 15th.
Eye Examination Form:
- Eye examinations must be conducted within twelve months prior to the first day of school; forms are due no later than October 15th. The student’s report card may be withheld until the eye examination form is submitted.
- Families may also submit a waiver of eye examination if they meet the identified criteria.
- Your school nurse may also have certificates for free eye examinations and glasses for children who qualify. Please contact our school to request this certificate.
Dental Examination Form:
- Dental examinations may be conducted within 18 months prior to the deadline.
- If a student fails to provide proof of a dental examination, the student’s report card may be withheld until:
- A dental examination waiver form is submitted
- A religious exemption form is submitted, or
- Proof that a dental examination will take place within 60 days after the deadline.
Allowable exemptions:
- Religious exemptions - Parents or legal guardians who have a religious objection to health, dental, and/or eye examinations; immunizations; or hearing and vision screenings may present a signed Certificate of Religious Exemption form.
- This form must detail the grounds for the objection and must list the specific immunizations or examinations that they are objecting to.
- The Certificate of Religious Objection form must also be signed by the appropriate health care provider.
- Updated religious exemption forms are required for whenever new or updated health forms are required.
- Medical exemptions - When immunizations are delayed due to a medical reason, a statement signed by the student’s health care provider must be provided.
- This statement should include the reason for the immunization delay and a catch-up immunization schedule.
Sports Physicals:
Students who participate in after-school sport programs must have a current sports physical on file. A current Certificate of Child Health Examination OR an IESA/IHSA pre-participation form will meet this requirement. The IESA/IHSA form is valid for 395 days.
Identified Health Needs:
Students with identified health needs may need to provide additional health forms prior to the first day of school. These may include forms for medication at school, asthma, seizures, diabetes, food allergies, or other guidelines that address the student’s specific health needs. Forms must be updated every school year.
